BSA Health and Medical form required for many O.A events!

Parts A and B of the BSA Annual Health and Medical Form #680-001   are to be completed and brought to all O.A. events not exceeding 72 consecutive hours.  Please pass this information on to everyone with your unit who might be planning to attend and Lodge and Chapter functions. 

Parts A and B of BSA Annual Health and Medical form #680-001 are to be completed and brought to all O.A. events not exceeding 72 consecutive hour. 

Part A - Medical information required includes a current health history and list of medications. No physical examination by a physician is required unless the event is more than three days. 

Part B - Includes the parental informed consent and hold harmless/release agreement as well as a talent release statement. This form is to be filled out by participants and parents or guardians and kept on file for easy reference.

    Your input is requested!

To better serve those in our Lodge, we’d appreciate hearing from all Tetonwana Lodge Arrowmen about your level of understanding of the various areas of the Order of the Arrow. This information will help us determine what training is needed, and also give us an idea of the topics or areas of interest our members would like to see offered at the annual Lodge Leadership Development (LLD) and other training sessions offered throughout the year. This information also helps identify those who are best qualified to help train others. Please fill out a “General Membership Questionnaire” form and bring it to the next lodge function.

How can I be of service to my fellow Arrowmen and the Lodge? 

There are a number of ways Arrowmen can serve their Lodge and fellow Arrowmen.  These include serving on the Tetonwana Lodge Ceremonies TeamExternal Link, serving as an Elangomat or Nimat, or serving as an OA Troop Representative.  Serving as an OA Unit Representative is a leadership position that fulfills a rank advancement requirement.  If interested in this leadership position fill out an OA Troop Representative registration form and give it to your chapter leadership.
