Medical Process

 

Each participant needs to submit an Annual Health and Medical Record, one that is specially bar-coded for each participant.  You will receive your medical record via email, it is critical that you:

DO NOT allow anyone else to use your form

DO NOT use anyone else’s form

DO NOT download a form Scouting.org or use a form without a specific bar code.

ONLY use the form sent to you that has your specific bar code

You will need to print the medical record and schedule a physical with a health care provider to get the form completed and signed, then return the form to the Council office.  The medical form needs to be complete, no empty spaces, signatures are properly filled out.

We have until March 1, 2010 to forward the medical forms to the Jamboree offices, they will be reviewed and accepted or denied.  If accepted, each individual will be notified by email, see the 2010 NSJ Medical Process for the denial process.
